A few things this procedure is not:

What the Published Research Actually Shows
Published scientific research supports that hyaluronic acid (HA) injections can increase penile girth and improve patient satisfaction in clinical settings, with generally manageable safety outcomes.
Clinical trials have directly evaluated HA fillers for penile girth enhancement. In a prospective, multicenter, patient- and evaluator-blinded randomized trial comparing HA to polylactic acid (PLA) fillers, both groups experienced significant penile girth increases at 24 weeks following injection. Men in the HA group showed a mean girth increase of approximately 22.7 ± 12.6 mm (about 2.3 cm), and both HA and PLA groups reported significant improvements in satisfaction with penile appearance and sexual life. A systematic review and meta-analysis that pooled data from four studies with 262-283 participants found that penile girth increased more with HA filler than with PLA filler (P = 0.01). Sexual satisfaction scores at 12 weeks were also significantly higher after HA augmentation compared with PLA. In summary, the peer-reviewed evidence indicates that HA penile fillers can produce increases in penile girth and improvements in satisfaction with sexual aesthetics, and that short-term safety profiles are acceptable in clinical practice. Larger controlled studies with extended follow-up beyond 24 weeks remain limited, and no high-quality long-term trials have yet established durability over many years.
The case reports that get shared on social media are alarming, and they should be. One of the key safety advantages of HA over permanent fillers or surgical implants is that it’s reversible. An enzyme called hyaluronidase can dissolve the filler if needed, whether due to an unwanted result, asymmetry, or a rare complication. Risks
No procedure is risk-free. And I’d rather you hear about the real risks from a provider than from a tabloid headline.
Common and expected: Swelling, bruising, and tenderness at the injection site. This is normal. It typically resolves within a few days to two weeks.
Uncommon but documented in the clinical literature: Nodule formation, mild asymmetry, filler migration, and localized infection. Published complication rates in controlled clinical settings range from roughly 1% to 4%, depending on the study. These are manageable. Nodules can be dissolved with hyaluronidase, infections respond to antibiotics, and asymmetry can often be corrected with a touch-up.
Rare but serious: The case reports Forbes mentioned are real. Severe infections progressing to sepsis and tissue necrosis requiring surgical intervention have been documented. But when you look at the details of those cases, the common threads are almost always the same: unqualified practitioners, non-medical settings, or non-HA substances being injected.
